HSC 6615 Foundations of Population Health

This interprofessional course focuses on preparing health professionals with the foundational skills needed to work in teams to effectively collaborate and coordinate care in population health management. Special emphasis will be on identification and stratification of populations at risk; health care and public health systems, evidence-based care, care coordination, patient and community engagement, and data analytics and reporting of outcomes. Key themes of interprofessional communication, collaboration, leadership, and professionalism will be ingrained throughout content. The course integrates components of both health care and public health systems such as access, health promotion, disease prevention, screening, and chronic care utilizing strategies to advance beyond care to overall wellness.
