Name, Address, or Phone Number Change
It is the students’ responsibility to ensure that their information in our system is current and accurate. When a student’s name, mailing address, permanent address, phone number, or other pertinent information changes, he or she must promptly notify the Enrollment Services Office or Office of the Registrar and fill out the appropriate form to reflect the change in our system. Necessary forms are available online, in Eagle Central, and at off-campus centers. Students are responsible for any delays in communications sent from CSI when an incorrect address is on file.
